Job Title: Family Liaison Officer/Transition Co-ordinator

Location: Reddish Hall, Stockport, Greater Manchester

Salary:  £18,000 per annum

Hours: 8.00am - 16:00pm

Contract: Permanent, Term-time plus 6 weeks

This is an exciting time to join Reddish Hall as we expand our provision with the opening of The Hive, our new vocational unit.

Reddish Hall School is an independent specialist day school for boys and girls aged from 5 to 18, catering for children with social emotional and mental health difficulties, with a high proportion of children who have a diagnosis of ASC.  Our dedicated team work together to ensure successful outcomes for pupils who often arrive at the school with a troubled view of education and life.

Purpose of Job:

  • To co-ordinate transition for new pupils into school and exiting pupils
  • To support pupils starting at Reddish Hall School through information sourcing, liaising with transition timetables and support with transport
  • Under guidance from the Head Teacher and the Senior Leadership Team, to

engage with vulnerable families in order to formulate a package of support

  • To act as a point of contact in school for families in need of support
  • To signpost for families and refer for support when appropriate
  • Liaise professionally with external agencies such as Housing, NSPCC, Sure Start,

Social Care, Early Intervention Teams, Passenger Transport, YOS and Police.

  • To complete appropriate referral forms to external agencies as and when required
  • To attend multi-agency meetings as appropriate.
  • To carry out home visits where necessary
  • To build effective relationships with families
  • To provide opportunities for families to engage with the school and build relationships before the child starts at the school
  • To signpost where families can gain support from agencies in their local area
  • To maintain regular contact with families of children receiving support to

encourage positive family involvement in the child’s learning

  • To act as a Designated Safeguarding Officer for the school and ensure all training is kept current and valid
  • To maintain record keeping in accordance with the policies and procedures in

place in school, including case studies

  • To support the CME (child missing from education) Lead and ensure systems are robust and utilised effectively. Including creating plans to support vulnerable students or those who are regularly missing, absent or late.
